CONDITIONS OF ENTRY - Women's Health reader survey

1. Information on "how to enter" and prizes form part of conditions of entry. It is a condition of accepting the prize that the winner must comply with all the conditions of use of the prize and prize supplier's requirements.

2. Entry is open to Australian residents only, except employees and immediate families of the Promoter. Enter through the website: Complete the survey at www.yahoo7.com.au/womenshealth and submit the results with the answer to the question "Tell us in 25 words or less what your dream is". All entries must include the required skill components to be eligible to win.

3. The Promoter is Pacific Magazines Pty Ltd, 35-51 Mitchell Street, McMahons Point, NSW, 2060 (ABN 16 097 410 896).

4. The winner will be the entry selected, having regard to creativity and originality of wit of words, at the Promoter's premises on 17/12/07.

5. The winner will be notified by mail and their name will be published online at www.yahoo7.com.au/womenshealth on 20/12/07.

6. Total prize value is up to $1,000 as at 14/11/07. 2 x major prizes: $500 voucher from Lonsdale clothing.

7. The judges' decision in relation to any aspect of the competition is final and binding on each person who enters. No correspondence will be entered into. No responsibility is accepted for late, lost or misdirected acceptances.

8. Prizes will be sent to the winner's nominated address as stated in their original entry. The Promoter and their associated agencies, and companies associated with this promotion will take no responsibility for prizes damaged or lost in transit. The prize is not transferable, exchangeable or redeemable for cash.

9. If the prize is unavailable, for whatever reason, the Promoter reserves the right to substitute the prize for a prize of equal or greater value, subject to State Regulation.

10. Prizes will be sent within 28 days of the draw date.

11. The Promoter may conduct such further draws on 15/03/08 at the same time and place as original draw in order to distribute any prizes unclaimed by this date, subject to any written directions given under Reg 37 of the Lottery and Gaming Regulations 1993 (SA) or other applicable legislation. Winners from any further draws will be published at www.yahoo7.com.au/womenshealth on 20/03/08.

12. If for any reason this competition is not capable of running as planned including infection by computer virus, mobile phone failure, line drop out, bugs, tampering, unauthorised intervention, fraud, technical failures or any other causes beyond the control of the Promoter which corrupts or affect the administration security, fairness, integrity or proper conduct of this promotion, the Promoter [subject to any written directions given under the Lottery And Gaming Regulations 1993 (SA) or other applicable legislation] reserves the right to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the promotion. The Promoter is not responsible for any problems or technical malfunction of any telephone network or lines, computer on line systems, servers, or providers, computer equipment, software, technical problems of the phone or of any phone network, or any combination thereof, including any injury or damage to participants or any other person's phone related to or resulting from participation or sending or receiving of any communication or of any materials in this competition.

13. In the case of the intervention of any outside act, agent or event which prevents or significantly hinders the Promoter's ability to proceed with the competition on the dates and in the manner described in these terms and conditions, including but not limited to vandalism, power failures, tempests, natural disasters, acts of God, civil unrest, strike, war, act of terrorism, the Promoter may in its absolute discretion cancel the competition and recommence it from the start on the same conditions, subject to any written directions given under the Lottery and Gaming Regulation 1993 (SA).

14. The Promoter assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of entries, and reserves the right to take any action that may be available.

15. The Promoter reserves the right to refuse to allow a winner to take part in any or all aspects of the prize, if the Promoter determines, in their absolute discretion, that a winner is not in the mental or physical condition necessary to be able to safely participate in the prize. It is a condition of accepting the prize that the winner may be required to sign a legal release in a form determined by the Promoter in its absolute discretion.

16. The Promoter collects entrants' personal information in order to conduct the promotion and notify winners. If the information requested is not provided, the entrant may not participate in the promotion. Entrants' personal information may be disclosed to State and Territory lottery departments and winners' names published as required under the relevant lottery legislation.
